Plastic Surgery Tasmania and Mr. Daniel Bunker, Plastic and Reconstructive Surgeon, are committed to protecting the privacy and security of any personal information obtained about individuals and recognise that, under the law, you have certain privacy rights in relation to information that you give to us.

We collect information from you for the primary purpose of providing quality health care. The information you provide to us is personal. However, not having it will restrict our capacity to provide you with a high standard of medical care.

Use of Your Information

The main reason we collect information from you is so we can assess, diagnose and treat your illnesses properly and be proactive in your health care needs. This means we will use the information you provide in the following ways:

Administrative purposes of this practice.

Billing purposes, including compliance with Medicare and Health Insurance Commission and Private Health Fund requirements.

Disclosure to others involved in your health care, including treating doctors, specialists and allied health practitioners outside this medical practice. This may occur through referral to other doctors or for medical tests and in reports returned to us following the referral.

Disclosure to other doctors or allied health practitioners within the practice for the purpose of continuity of patient care.

Disclosure to visiting doctors and medical students attached to this practice for the purpose of patient care and teaching.

Disclosure for research and quality assurance activities to improve individual and community healthcare and practice management. Usually, no individual names will be recorded but if this is not the case, you will be informed when such activities are being conducted and given the opportunity to 'opt out' of any involvement.

For quality review, medical records may need to be assessed by a visiting doctor.

Disclosure to insurer, employer or solicitor when appropriate.

Marketing and communications purposes, including using information collected through advertising enquiries to provide you with relevant information about the procedures or services you have expressed interest in.​

You are not obliged to provide any information requested by us, but my failure to do so might compromise the quality of the healthcare and treatment given to me.

You have the right to access the information collected about me, except in some circumstances where access might be legitimately withheld. I understand I will be given an explanation in these circumstances.

There are some circumstances where information needs to be disclosed without consent, such as:

An emergency.

Where disclosure is necessary to lessen or prevent a serious and imminent threat to an individual's life, health or safety, or is a serious threat to public health or safety.

Where disclosure is a legal requirement, e.g. a communicable disease, suspected abuse, subpoena to court.
