Preservé is a new technique requiring specialised training for accreditation. Mr Dan Bunker is the first and currently the only surgeon accredited to perform this technique in Tasmania and one of only a small number of accredited surgeons in Australia.
What is Preservé®?
Preservé is a revolutionary breast augmentation technique developed by Motiva that combines a tissue-preserving surgical approach with specifically designed instruments and the Motiva SmoothSilk® Ergonomix2® implant. The technique is based on three principles:
1. Create a pathway with minimal tissue disruption
A specialised instrument is used to create a controlled pathway through the breast tissue rather than extensively dissecting a conventional implant pocket.
2. Gently expand the tissues
A specialised balloon is introduced and gradually inflated to gently separate and elongate the tissues, creating the space required for the implant with maximal preservation of the supporting circummammary ligaments.
3. Insert the implant through a small incision
Once the tissues have been appropriately prepared, the implant is inserted through a small incision, typically positioned within the breast fold. The objective is to work with the patient's existing anatomy rather than extensively disrupting it.

The Preservé® approach
Traditional breast augmentation involves creating an implant pocket through surgical dissection. Depending on the patient's anatomy and the chosen technique, the implant may be positioned above or below the pectoral muscle. Preservé is designed to create the implant space through controlled tissue separation and expansion, while preserving the pectoral muscle and minimising disruption to the surrounding tissues.
The result is a fundamentally different approach to creating space for the implant.
The potential benefits of Preservé®
Preservation of the pectoral muscle
The pectoral muscle is not cut or released during the Preservé procedure. This may be particularly attractive to patients who are physically active or regularly undertake upper-body exercise. Preserving the muscle may also avoid some of the movement-related effects associated with implants positioned beneath the pectoral muscle, including animation deformity.
Less tissue dissection
Preservé is designed to minimise the amount of surgical dissection required to create the implant space. Less tissue disruption may contribute to reduced postoperative discomfort, swelling and bruising, although individual recovery varies.
Small incision
The procedure is performed through a relatively small incision, usually positioned within the breast fold.
Rapid recovery
One of the most significant potential advantages of Preservé is the speed of recovery. Because the technique minimises tissue disruption and preserves the pectoral muscle, appropriately selected patients may return to normal activities considerably sooner than with some conventional breast augmentation techniques. For some patients with desk-based occupations, return to work may be possible as early as the following day. This does not mean that the tissues are completely healed, and patients will still need to follow postoperative instructions regarding exercise and other physical activities.

Can Preservé® be performed under local anaesthetic?
A further feature of the Preservé approach is that, in appropriately selected patients, the procedure can be performed using local anaesthetic with intravenous sedation. This may allow patients to avoid a general anaesthetic, although the appropriate anaesthetic technique is determined on an individual basis in consultation with the anaesthetist. Whether local anaesthesia and sedation are appropriate will depend on the patient's health, anatomy, procedure and individual circumstances.

Preservé may not be suitable for patients who:

Require significant reshaping of the breast
Recovery following Preservé®

The reduced tissue disruption associated with Preservé is designed to facilitate a rapid recovery.

Patients can generally expect:

Day of surgery: The procedure is performed as a day procedure. Patients are monitored following surgery and can generally return home the same day.

First few days: Some discomfort, swelling, tightness and bruising are expected. These symptoms are generally managed with simple analgesia.

Return to work: Patients with sedentary or desk-based occupations may be able to return to work very quickly, with some appropriately selected patients returning the following day.

Exercise: A graduated return to physical activity is required. The timing will depend on the individual patient and the nature of the exercise.

Longer term: The breasts will continue to settle and change in appearance as postoperative swelling resolves and the tissues adapt to the implant.

Recovery varies between patients, and individual advice will be provided following surgery.
Important information
Preservé® is not suitable for every patient. Suitability is determined following an individual assessment of the patient's anatomy, breast tissue, desired breast size and surgical goals. As with all breast implant surgery, there are potential risks and complications, including infection, bleeding, changes in sensation, capsular contracture, implant malposition, implant rupture and the potential need for further surgery. Breast implants are not lifetime devices. Preservé is a relatively new technique and, as with all emerging surgical techniques, ongoing clinical experience and longer-term evidence will continue to inform its use.
